[Abstract] Objective To evaluate percutaneous transluminal septal myocardial ablation (PTSMA) treatment of hypertrophic obstructive cardiomyopathy (HOCM) in the medium-term efficacy. Method in August 1998 on October -2001 HOCM patients were enrolled in 119 lines PTSMA, of which 34 cases were followed up nearly 2 (2 0.5) years. In the six months before and after ablation (5-7 months) to 2 years line echocardiography ventricular septal thickness and left ventricular outflow tract width. Results 34 patients were followed up after 2 years the average septal thickness (16.1 4.8) mm, [pre (23.9 6.0) mm, after six months (18.8 4.1) mm], the average left ventricular outflow tract width (15.1 5 .3) mm, before operation [(6.6 2.3) mm, after six months (12.1 3.3) mm]. Conclusion PTSMA can significantly reduce septal thickness, increased left ventricular outflow tract width, increasing effect with time.

October 2001,119 patients with HOCM underwent PTSMA by Sigwart method. Thirty four patients among them were follow-up2 (2 0.5) years. The tickness of septal myocardium and width of left ventricular outflow-tract (LVOT) were measured before and 6 months and 2 years after PTSMA. Results The thickness of the septal myocardium reduced from (23.9 6.0) mm before ablation to (18.8 Soil 4.1) mm at 6 (5-7) month after PTSMA, and (16.1 Soil 4.8) mm at 2 years after PTSMA. The width of LVOT expanded from (6.6 soil 2.3) mm before ablation to (12.1 Soil 3.2) mm at 6 month and (15.1 5.3) mm at 2 years after PTSMA. Conclusion Two years follow-up data showed that PTSMA could significantly reduce the thickness of septal myocardium and expand the width of LVOT.

Discussion

Except HOCM valve disease must be diagnosed with hypertension and other diseases caused by left ventricular overload.
HOCM variety of clinical manifestations, severity, course, outcome, prognosis varies. Patients may be asymptomatic, sustained in a steady state or in the 5 to 10 years to deteriorate rapidly. Most patients can be maintained for about 10 years of stability, can worsen some symptoms and some may even be improved in clinical symptoms. 3% annual mortality of adult children of about 6%.
